I (which expression includes the student and the parent/guardian) hereby apply to enrol in the program indicated on the Enrolment Application ('the Application'). I agree that on confirmed acceptance of the Application by OP Centro Tuition, a legally binding Contract of Enrolment ('the Contract') will be formed and further I agree to abide by the following terms and conditions:

1. Availability
The completion of this application does not guarantee enrolment in the program.
Free summer enrichment programs are only available to students living in or attending school at Brisbane southern suburbs and places are subject to availability.

2. Conduct and Behaviour
OP Centro Tuition reserves the right, in the exercise of its sole judgement, to refuse entry to or evict from its premises any person who behaves inappropriately.
Inappropriate behaviour and misconduct includes, but is not limited to:
i. Any breach of Commonwealth or State law;
ii. Any harassment or bullying of other students or of staff;
iii. Any behaviour which impacts detrimentally on other students;
iv. Any act or failure to act that endangers the safety or health of any other person;
v. Acting in a way that causes students or staff or other persons to fear for their personal safety, e.g. violent or threatening behaviour;
vi. Acting in a way that causes wilful damage to OP Centro Tuition or staff or other student's property;
vii. Being under the influence of drugs or alcohol;
viii. Cigarette smoking.

3. Refund Policy
There are NO credits or refunds available at Op Centro Tuition for either missed classes or cancellation of course lessons.
